What to do if Memory card does not exist?

I just reformatted my memory card, then I tried to re-install some application from Samsung Kies using my phone and it happened but one of the apps had an error, so I tried to install it again but my phone suddenly turned off and got disconnected. I was trying to connect it again but memory card didn't read anymore. So I used a card reader and inserted my memory card in there, unfortunately "Please insert a disk into drive D:" appeared. Computer couldn't read it. Then I tried to insert it back to my phone and I was trying to format it but "Memory card does not exists" appeared. What should I do? Could I still fix it? it's 16GB so really felt so upset.
